Catasys provides specialized behavioral health management services to health plans, employers and unions through a network of licensed and company managed health care providers. Catasys’ integrated program, OnTrak is designed to address substance dependence as a chronic disease as well as other identified client issues such as stress, depression, anxiety, smoking cessation, and weight management. The program seeks to lower costs and improve member health through the delivery of integrated medical and psychosocial interventions in combination with long term care coaching and online tools to aid members in improving their overall health.

The Position:

 The Care Coach is responsible for engagement, enrollment, and coordination of care to improve the health and quality of life for members of the Catasys OnTrak program.  Care Coaches utilize clinical and functional assessments of client progress, employ targeted intervention and education, and coordinate care pertaining to clients’ identified issue(s).  The Care Coach utilizes Motivational Interviewing techniques to elicit active member participation.  In addition, the Care Coach is responsible for the coordination of member care throughout the treatment continuum:  medical intervention, relapse prevention, treatment of co-morbid conditions, and issues identified through the individual care plan.

Responsibilities:

 Uses exceptional written and verbal communication skills to engage, enroll, and maintain members in active participation in the OnTrak program as well as to coordinate member care.

  1. Conducts superior motivational interviewing, enhancement, and relapse prevention guidelines in telephonic communication with clients.
  2. Demonstrates superior knowledge of the Catasys OnTrak treatment program and usage of the eOnTrak electronic documentation platform.
  3. Provides exemplary customer service to and member-centric focus for, all clients.
  4. Communicates or requests information from providers in complete compliance with confidentiality standards and in a manner that generates cooperation. 
  5. Complies with Catasys policies and procedures, including training, audits, alert response timeliness, intervention and follow-through, supervision receptiveness, as well as other Catasys related clinical/medical services.
  6. Complies with Catasys Human Resource policies and procedures.
  7. Supports and meets any Catasys accreditation, auditing, contracted stakeholder reporting expectation. 
  8. Works jointly with the Clinical Care Coordinator to support member referral, engagement, and enrollment into the OnTrak program.
  9. Supports enhancement of the eOnTrak electronic documentation platform by providing feedback to the development team and performing ad hoc system testing as indicated.

Requirements:

 Minimum: Registered Nurse OR Masters degree and licensure in professional counseling, clinical social work, or marriage family therapy. Candidates must have a minimum of three years of direct clinical experience working with clients with substance dependence and co-morbid diagnoses, including common chronic disease states. Must have excellent written and verbal communication skills and demonstrate proficiency in the utilization of Microsoft Office features. Experience with electronic medical record documentation is a plus.  Experience utilizing Motivational Interviewing techniques preferred.

APRN Advanced Practice RN with CNS/NP in psych Mental Heath and extensive leadership experience in psychiatric Nursing or RN with Masters in related field is given preference for this position. BS in Nursing is considered if experience is comparable.

THE PROGRAM
Correctional Health Services - Mental Health
This program provides mental health services to an adult inmate population housed within five separate correctional facilities. The program is staffed with a multi-disciplinary team of dedicated mental health professionals who are committed to providing quality health care to inmates in a 24 hour locked facility environment. Incumbents may in the course of their job duties, be providing routine psychological evaluation
or crisis intervention to clients who are depressed, agitated or psychiatrically unstable and must exercise social and clinical skill in eliciting information necessary for the purpose of evaluating symptoms and determining appropriate treatments and/or referrals.
THE POSITION
Clinically supervised by the psychiatrist. Responsible for psychiatric assessment and treatment including, case management, medication prescribing and management, crisis intervention, suicide prevention and to do other work as required.
Responsibilities include but are not limited to:
  • Independent assessment and evaluation of inmates and detainees including dual
  • diagnosis and seriously mentally ill (SMI) clients.
  • Case management/rotation through all adult jails
  • Medication evaluation, treatment and follow-up
  • Weekend coverage of the jail inpatient psychiatric unit
  • Consulting with psychiatrist as needed
  • Other duties as assigned.

 MINIMUM QUALIFICATIONS

       License

  • Possession of a valid Registered Nurse License issued by the California Board of Registered Nursing.
  • Possession of certificate that nurse has met the requirements for Nurse Practitioner issued by the California Board of Registered Nursing.
  • Possession of a valid DEA license for Controlled Substances Level II.
